Default Problem with Cigarette Lighter

I have an AU Falcon with a very fussy cigarette lighter

It doesn't work with either the car fridge, the GPS Receiver, nor the Mobile Phone Charger cigarette lighter adapter ... BUT up until now, it HAS worked (with a bit of jiggling) with a Cigarette lighter DOUBLE adapter, that I've plugged all my accessories into

Unfortunately the Double adapter broke, so I've replaced it with a new one (same model) and it doesn't work. At the same time as this broke (a coincidence, I'm sure), the following piece FELL OUT of the cigarette lighter ..
(Smal black circle 15mm in diameter, steel insert)

My questions are ...

[1] Does this piece BELONG in the cigarette ligher or has it dropped off a plug somewhere ? (Yes, I checked all the plugs, nothing seems to be missing and the part doesn't APPEAR to want to stay there when I try and stuff it back in .... Yes, I'm careful not to start a fire)

[2] Any ideas why it wouldn't be working ? (Yes, I've checked the fuse and it looks fine)

That picture is a bit small and blurry to actually se what that is.

To remove cig lighter, you need to remove the radio, and the radio surround/facia. Then it's easy to remove the old cigarette lighter and put in a new one (generic $7 one from superchrap will fit). Do a search on facia removal or radio removal to find out how to do that.
