Question New Keys and Remotes

Could someone explain (or link) what exactly is the go with all those crazy costs associated with getting new sets of keys and remotes. More importantly a technical explanation of how the whole system really works.
I've got an idea, but I don't know where it came from. I'll share so as to provide a springboard for replies.

/CAR has 'unique' a physical code as well as a digital code.
|-ICC contains 5 (or so) memory banks which store REMOTE codes which are authorised to operate vehicle.

/KEY has physical code as well as a transponder.
|-PHYSICAL code is old fashioned cut similar to pin tumber lock
|-TRANSPONDER must be programmed by car to transmit the correct code to unlock the car. (else ignition is disabled although physical entry into vehicle is gained)

/REMOTE has unique non-programmable code.

How is it different when you only have one key/remote left?
The car can be easily programmed to recognise new remotes, regardless of how many you have. Keysmiths can program a new key if you have two Keys, but Ford must do it if you only have one left (Is that right? Why so?)

Maybe I should have waited till morning to post.

p.s. What does ICC stand for? Integrated Car Computer?
